More fixes for round-trip tests of HTML reader.

We exclude tables that have default widths but non-simple
content, as these can't really round-trip.

Exclude tables with cells with line breaks because they don't
currently round-trip. (Table goes from being simple to having
explicit widths.)

HTML reader: read data-foo attribute into foo.
...
The HTML writer adds the `data-` prefix for HTML5
for nonstandard attributes. But the attributes are
represented in the AST without the `data-` prefix,
so we should strip this when reading HTML.
Closes #5392 .

Add missing copyright notices and remove license boilerplate ( #5112 )

Quite a few modules were missing copyright notices.
This commit adds copyright notices everywhere via haddock module
headers. The old license boilerplate comment is redundant with this and has
been removed.
Update copyright years to 2019.
Closes #4592 .

Some of the round-trip tests are set to do a maximum of 25
trials when the default value is given.
