c423dbb5a3 Use -XNoImplicitPrelude and 'import Prelude' explicitly. 
This is needed for ghci to work with pandoc, given that we
now use a custom prelude.

Closes #2503.

464b174d0f Fixed reference slides. 
The Div container around references messed up the procedure
for carving a document into slides.  So we now remove the surrounding
Div in prepSlides.

Changed name to revealjs (from reveal_js).
Set revealjs-url template variable to 'reveal.js' by default.

6b53a905c4 Added basic support for reveal.js. 
Support unordered and ordered lists with "fragment" elements.

Modified by JGM to remove the --reveal_js-url command-line option.
Instead use -V reveal_js-url=... as with slidy and the other slide
formats.  Also cleaned up the list code in the HTML writer.

d599c4cdab Added Attr field to Header. 
Previously header ids were autogenerated by the writers.
Now they are generated (unless supplied explicitly) in the
markdown parser, if the `header_identifiers` extension is
selected.

In addition, the textile reader now supports id attributes on
headers.

527be1ddf2 Don't generate empty H1 after hrule slide breaks. 
We now use a slide-level header with contents [Str "\0"] to mark
an hrule break.  This avoids creation of an empty H1 in these
contexts.

Closes #484.
