b806bff5b4 LaTeX reader: fix bugs omitting raw tex. 
The default is `-raw_tex`, so no raw tex should result
unless we explicitly say `+raw_tex`.  Previously some
raw commands did make it through.

Closes #4527.

013fd1c6b6 Make sure \write18 is parsed as raw LaTeX. 
The change is in the LaTeX reader's treatment of raw commands,
but it also affects the Markdown reader.

d17f0dab84 LaTeX reader: better support for subfigure package. 
A figure with two subfigures turns into two pandoc
figures; the subcaptions are used and the main caption
ignored, unless there are no subcaptions.

Closes #3577.
